Can be microscopic or macroscopic due to an acute event or gradual over time. May be as obvious as a prior large heart attack with obvious focal scarring or as subtle as long standing hypertension causing diffuse microscopic damage to the entire heart muscle (leading to a stiff heart - HFpEF) or kidneys, for instance.
